A U.S. passport allows U.S. citizens, including all U.S. citizen minors regardless of age, to travel internationally. The U.S. passport is also recognized as a document that proves one’s U.S. citizenship. Therefore, a lost or stolen passport has to be replaced as soon as possible. It is important to report and replace a lost or stolen passport.
There are three steps in reporting a lost or stolen child passport.
Step 1. Complete Form DS-64, Statement Regarding a Lost or Stolen Passport.
Complete Form DS-64 as best as you can, writing legibly and in black ink.
Step 2. Sign and Date Form DS-64, Statement Regarding a Lost or Stolen Passport.
Form DS-64 must be signed and dated on page 1 of the form.
Step 3. Submit Form DS-64 with a Completed Form DS-11, The Application For A U.S. Passport
If you want the lost or stolen child passport to be replaced, submit a completed Form DS-11 and Form DS-64 in person to an Acceptance Facility or Passport Agency.
