Form DS-64, Statement Regarding Lost or Stolen Passport Since United States citizens are generally not allowed to have more than one valid passport, applicants who are not able to show their old passport when applying for a replacement passport must fill out and submit form DS-64, Statement Regarding a Lost or Stolen Passport. Anyone who uses a passport that has been reported lost or stolen to, to enter the U.S., may be detained. There are three steps in reporting a lost or stolen passport.

Step 1. Complete Form DS-64, Statement Regarding a Lost or Stolen Passport:

Step 2. Sign and Date Form DS-64, Statement Regarding a Lost or Stolen Passport:
Form DS-64 must be signed and dated on page 1 of the form.

Step 3. Submit Form DS-64 with a Completed Form DS-11, The Application for A U.S. Passport:

If you want your lost or stolen passport; submit a completed Form DS-11 and Form DS-64 in person to an Acceptance Facility or Passport Agency.
