Form DS-64, Statement Regarding Lost or Stolen Passport Since United States citizens are generally not allowed to have more than one valid passport, applicants who are not able to show their old passport when applying for a replacement passport must fill out and submit form DS-64, Statement Regarding a Lost or Stolen Passport. The information provided in the statement will be stored in a consular database over lost and stolen passports. Anyone who uses a passport that has been reported lost or stolen to, to enter the U.S., may be detained. A U.S. citizen who loses his/her passport should report it, using form DS-64, to prevent identity theft. If a passport that is reported lost or stolen is later found, it must be reported and turned in to be cancelled.

There are three steps in reporting a lost or stolen passport.

Step 1. Complete Form DS-64, Statement Regarding a Lost or Stolen Passport:
Complete Form DS-64 as best as you can, writing legibly and in black ink. If you do not know the answer to a question, try to provide an answer as best as you can.

Step 2. Sign and Date Form DS-64, Statement Regarding a Lost or Stolen Passport:
Form DS-64 must be signed and dated on page 1 of the form.

Step 3. Submit Form DS-64 with a Completed Form DS-11, The Application for A U.S. Passport:

If you want your lost or stolen passport; submit a completed Form DS-11 and Form DS-64 in person to an Acceptance Facility or Passport Agency.
